Trump's $500 ACA Refund Checks Land Weeks Before Midterms

About 950,000 Americans are opening envelopes containing $500 checks signed by President Trump — one-time Affordable Care Act refunds that began landing September 30, just five weeks before the November 3 midterms. The administration says the money returns a Biden-era "overcharge." Democrats call it an illegal vote-buying scheme.

Who gets the $500

The checks go to ACA enrollees in the 30 states that use the federal exchange (HealthCare.gov) who buy coverage without federal subsidies — mostly people earning above roughly 400% of the federal poverty line, about $62,600 for an individual. Families with multiple eligible members can receive multiple checks. Residents of the 20 states running their own exchanges — including California, New York, and Pennsylvania — plus Washington D.C. are excluded. Texas has the most recipients, followed by Florida with 127,900 and Ohio with 65,700.

Each check arrives with a signed letter from Trump: "That money belongs to hard-working Americans, not the Government, and now I'm returning it to you." The program was announced September 10 in a White House video posted on X, timed with the GOP midterm convention in Dallas.

The administration's case — and the pushback

The White House argues Biden-era exchange user fees, charged to insurers and passed into premiums, exceeded the cost of running the exchange — leaving a surplus of at least $500 million that recipients were "wrongly overcharged." Experts dispute that framing: the surplus piled up partly because Trump's first administration slashed enrollment and marketing spending.

Democrats question the legality — the payments went out without congressional approval. Senator Ron Wyden was blunt: "If you want to BUY midterm votes, maybe start by not gutting everyone's health care in the first place." Health analysts at KFF found some enrollees' premium spikes far exceed the $500 payment.

Part of a bigger pre-midterm money push

The ACA checks are one piece of a wider pre-election cash offensive. More than 20 million Medicare seniors began receiving deposits of about $90 each from the Medicare Improvement Fund in early October. And Trump has separately promised a $5,000 "Trump dividend" to every adult American — if Republicans keep Congress — a pledge with an estimated cost above $1.2 trillion that would itself require Congress to act.

The takeaway: $500 checks with the president's signature, arriving five weeks before voters decide Congress — whether it's restitution or vote-buying depends entirely on which side of the aisle you're on.
